If you have recently suffered an injury that resulted in a diagnosis of being disabled, then one of the things that you will need to do is file a disability claim with your insurance company.   In the event that your long term disability insurance provider is Aetna Insurance, then you will definitely need to complete the disability claims form packet in order to file an Aetna long term disability claim.  The disability claims form packet consists of 10 pages that you will need to fill out completely and accurately.

Completing the Aetna Disability Claims Form Packet

  • The first form of the packet is called the “Enhanced Disability Claim Application –Employee Request for Information.”  This particular form is the official claim notice document.  You must complete all of your employee information which you will find on the lower part of the form. 
  • The second form details your capabilities and limitations as assessed by your physician. 
  • The third form goes hand-in-hand with the second floor in that it details the history, diagnosis and treatment of your disability as per the prognosis of your doctor.

 Once you get the second and the third forms from your doctor you should submit them directly to Aetna, separate from the rest of the packet.  These two forms are crucial for the claims representative to determine to either accept or deny your disability claim.

Additional Forms

The next few forms enable you to disclose information on any other income that you may have or expect to have, as well as authorization forms for various agencies (i.e. Social Security Administration) to release information on you and any Social Security checks that you may be entitled to. 

Additionally, there is a form in the pack that enables Aetna to get information on your educational background and work history.  You are in essence, giving Aetna permission to do a full background check on you and your health history in order for them to make an assessment on whether or not to grant your disability insurance request.

Once you have submitted your claims packet it would be a good idea to follow-up consistently with them.  Also, while your disability claim is being processed, you should speak to a disability insurance claim advocate just to get an idea of the expected outcome of your claim.  It may be a good idea to consult with a disability litigation attorney.  He or she will better be able to provide you with advice and insight on what to do should your claim be denied.  It is best to get your ducks lined up so that will be prepared no matter what.
