To apply, you will need to complete an application for Social Security Benefits and a Disability Report. One way to speed up the process is to provide the SSA with all the information they need, including your Social Security number; birth or baptismal certificate; names, addresses, and phone numbers of your treating medical providers, and dates of service; names and dosages of all your medication; copies of medical records that you have in your possession; lab and test results; a summary of where you worked and the type of work you performed; and a copy of your most recent W-2 form, or if you are self-employed, a copy of your most recent tax return. If you cannot get all this information together, Social Security can help you get it.
