You should apply for benefits as soon as you become disabled, as it can take the Social Security Administration three to five months to process applications. To apply, you will need to complete an application for Social Security Benefits and a Disability Report. One way to speed up the process is to provide the SSA with all the information they need, including your Social Security number; birth or baptismal certificate; names, addresses, and phone numbers of your treating medical providers, and dates of service; names and dosages of all your medication; copies of medical records that you have in your possession; lab and test results; a summary of where you worked and the type of work you performed; and a copy of your most recent W-2 form, or if you are self-employed, a copy of your most recent tax return. If you cannot get all this information together, Social Security can help you get it.
