The basic steps for filing your LTD claim with AETNA are as follows 

  1. Contact AETNA for a Long Term Disability Claim packet, which contains all the paperwork you'll need for your claim. There are a total of ten forms, all of which need to be filled out completely and correctly. They range from your official notification to Aetna to authorization forms that allow Aetna to contact your employer, your doctors, and whoever else they need to contact in order to verify your claim.

  2. Complete the forms to the best of your ability – this is where a lawyer can come in handy, or a phone call to Aetna. Make sure you comprehend what is being asked and answer with all required information and/ or proof.

  3. Return the forms to Aetna, with the exception of the statement form that is meant for your doctor. Forward this form to your doctor, who will be required to fill it out and send it on to Aetna on your behalf.
