What do I need from my doctor to file for disability benefits?

Answer:

The Social Security Administration provides benefits for those that are, or become, disabled.  Disability benefits provide monthly assistance to supplement an income.  To apply for disability benefits, submit an application to your city’s social security office.  Include in your application for disability benefits all contact information of your doctor or someone who is very familiar with your medical condition.  In addition, you need to submit all information about doctors you have seen, including dates of treatment and your patient identification number.  The application to receive disability benefits requests information regarding all medications you are taking and the names and dates of all medical tests you have received.  Additionally, the Social Security Administration requests that you provide information regarding your employment history and any workers’ compensation claims filed, and benefits received, in the past.

After you submit an application for disability benefits, the Social Security Administration will investigate your claim.  It is not necessary to submit anything from your disability doctors; however, a note from your doctor describing your condition may be beneficial.  If you do include a note from your doctor, ask your doctor to include detailed information about your condition.  The note should also describe how your condition prevents you from working, and why it is necessary for you to receive disability benefits.
